We are currently seeking a motivated and experienced Home Care Coordinator/Community Health Nurse that enjoys variety and diverseness in their nursing practice.
Job Summary:
Working within a multidisciplinary team and under the supervision of the BPFN Health & Nursing Advisor / Health & Wellness Health Director, the Home & Community Care Coordinator/Community Health Nurse is responsible for the provision of home care/in-home assisted living, public health, and direct nursing treatment services. The Home Care Coordinator component delivers intake, assessment, coordination, monitoring/evaluation and support services to clients and direct service staff of the home care/in-home assisted living programs. The community care and health component includes Public Health, Disease Prevention, Health Promotion and Community Development initiatives.
Qualifications:
- Registered nurse(RN) from a recognized institution and eligible for registration with the College of Registered Nurses (CRNM) and the Association of Registered Nurses of Manitoba (ARNM). A Baccalaureate degree in nursing (BN or BScN) and /or a certificate in Immunization or Community Health Nursing is an asset.
- Experience working in a community health setting and/or social program preferred e.g. Home Care, Public Health, Community Health, Jordan’s Principle, etc.
- Previous administration, leadership, and supervisory experience considered an asset.
- Working knowledge and experience with First Nation communities, culture, values, and beliefs.
- Excellent interpersonal and communication (verbal and written) skills.
- Physical ability to meet the demands of the job.
- Ability to work independently as a member of a multi-disciplinary team.
- Strong organizational, time management, multitasking, problem solving , conflict resolution, decision making, flexibility and critical thinking skills.
- Proficient in computer skills e.g. Microsoft MS office and Outlook programs or willing to train.
- Satisfactory criminal, vulnerable sector child and adult abuse registry checks.
- Valid drivers’ licence and reliable vehicle with all purpose insurance of at least 1,000,000.00 third party liability.
